Adaptec has announced a limited recall of its DPT SmartRAID Decade VI
Ultra160 SCSI RAID controllers after discovering errors in the Qlogic 12160A
SCSI chips that were used to manufacture the controller cards.
This precautionary recall affects a limited number of controllers that were
produced prior to May of 2000. The flawed Qlogic chips could send incorrect
information to the RAID controller, which can, in some cases, lead to SCSI
bus hangs and parity errors.
Limited Number of Components Affected
The DPT Decade Ultra160 SCSI RAID controllers recalled include those with:
a model number of PM-1564U3 or PM-1564U3-2U3;
AND a product serial number between 03-000001 and 03-005599. (Note: serial
number is located on the lower right face of the RAID controller)
Most of these products were distributed through value-added resellers and
systems integrators.
Replacing the Product...
If you have purchased one of the recalled DPT Decade RAID controllers,
Adaptec will replace it (while supplies last), or offer a comparable
Ultra160 SCSI RAID Controller. In addition, Adaptec is offering customers
their choice of up to $300 cash for installation or an additional Adaptec
Ultra160 SCSI RAID controller (a $450 value).

You will need to supply the serial number for the product(s) (found on the
lower right corner on the face of the controller card), shipping address,
contact name and telephone number. We will also ask for a credit card number
(Visa, M/C, AMEX) to ensure we receive our recalled product back within 30
days of shipment.
All recalled product must be returned in the original packaging material or
equivalent to prevent damage during shipment. The cable must also be
returned with this product.
